What should I wear the day of my procedure?
Please wear loose, simple, comfortable clothing and comfortable walking shoes. Leave all jewelry at home. If you are having a pain injection, wear elastic loose fitting pants and avoid wearing jeans or pants with metal around the waist area, such as zippers, grommets or buttons. Please remember to leave all valuables at home. Consult with your physician for specific instructions.
Do i need to fill out anything prior to my appointment?
You are not required to, but we have general patient forms in our patient forms section where you can fill out the forms online and print them out. Bringing the filled out forms with you will decrease the wait time to get the care from our physicians.
What information should I bring with me when I come to ARC Healthcare?
All first-time patients need to bring an up-to-date medical insurance card and a government issued photo ID. It is helpful to your physician if you can bring copies of any pertinent prior lab work or imaging studies. If you take medicines, please also be prepared to discuss the exact names and dosages of the medicines that you are taking.
